For Care Credit health financing options, please visit Care Credit. You can apply online prior to visiting our hospital or you are welcome to contact a staff member anytime. If applying at our office, please bring 2 valid forms of ID such as a state-issued Driver’s License or Passport, AND a valid credit card with expiration date. These are REQUIRED to apply. Clients will be denied the ability to use this form of payment or apply if these forms of ID are not presented. Please be advised, due to new Care Credit regulations, clients applying at our office will need to apply on their own via phone or internet.
